We are proud to serve those with developmental disabilities who participate in the Arizona Division of Developmental Disabilities (DDD) program. These DDD services promote independence and inclusion within the community for eligible members with developmental disabilities and their families, in the least restrictive home and community-based settings. We provide needed support to residents of the state of Arizona who meet the following eligibility requirements:

  • Are at risk of having a developmental disability (up to age six) or, for people over the age of six years with a diagnosis of epilepsy, cerebral palsy,
    cognitive/intellectual disability or autism
  • The disability occurred prior to the age of 18
  • Has substantial functional limitations in three of the seven major life areas, which include:

1. Self-care: Eating, hygiene, bathing, etc.
2. Receptive and expressive language: Communicating with others
3. Learning: Acquiring and processing new information
4. Mobility: Moving from place to place

5. Capacity for independent living: Needing supervision or daily assistance
6. Economic self-sufficiency: Being able to financially support oneself.
7. Self-direction: Managing personal finances, protecting self-interest, or making independent decisions which may affect well-being

VIEW SERVICES BELOW

Titles VI and VII of the Civil Rights Act of 1964 (respectively “Title VI” and “Title VII”) and the Americans with Disabilities Act of 1990 (ADA) Section 504 of the Rehabilitation Act of 1973 and the Age Discrimination Act of 1975, CJPS Home Care) prohibits discrimination in admissions, programs, services, activities or employment based on race, color, religion, sex, national origin, age, and disability. CJPS Home Care must make a reasonable accommodation to allow a person with a disability to take part in a program, service, or activity. Auxiliary aids and services are available upon request to individuals with disabilities. For example, this means that if necessary, CJPS Home Care must provide sign language interpreters for people who are deaf, a wheelchair accessible location, or enlarged print materials. It also means that CJPS Home Care will take any other reasonable action that allows you to take part in and understand a program or activity, including making reasonable changes to an activity.
